Transcript Requests
Each and every school you apply to will require an official transcript to be sent as part of the application. Please note that you must fill out and submit the attached form below for EACH school you submit an application to. Official transcripts will be mailed directly to the college. Some colleges require an additional form be sent with the transcript, so check your application packet and submit the form with appropriate instructions to the College Counselor.

Counselor Recommendations
The Secondary School Counselor Report form (found on the common application) should also be submitted to Ms. Cappadona along with the Transcript Request Form. Please provide a copy of your resume/activity sheet as well. All this paperwork will then be sent directly to the college/university from the College Counseling office.

Teacher Recommendations
The student should submit the following to the teacher:

• The teacher recommendation form (more typical with the common application)
• A stamped and addressed envelope, full legal name, social security number, and the date by which the recommendation must be received by the college
• A resume/activity sheet

Teachers will mail these directly to the college/ university. It is a good idea to check with the teacher to see if the letter was sent, and be sure to show your appreciation. Ultimately the responsibility is yours.

Mid-Year Reports
Some schools require a mid-year report. It is your responsibility to know which school requires this report. After completing all the relevant questions, give this form to Ms. Cappadona with envelopes addressed to each institution that requires a Midyear Report.

Final Transcripts
The college that you plan to attend will require a final transcript to verify your graduation. Final transcript requests will be announced in the Spring (April/May). It is your responsibility to notify the College Counselor where you need your Final Transcript sent.
